Overview
The immune system is the body's defense network, designed to identify and eliminate foreign invaders such as bacteria, viruses, and fungi. A critical feature of a healthy immune system is self-tolerance—the ability to distinguish the body's own healthy cells ("self") from foreign entities ("non-self"). Autoimmunity arises from a failure of this self-tolerance mechanism. The immune system incorrectly perceives normal body components as threats and launches an attack against them, leading to inflammation and tissue damage.
This misguided assault is carried out by the same tools the immune system uses to fight genuine pathogens: antibodies and T-cells. In autoimmune conditions, the body produces "autoantibodies" that target self-antigens on the surface of its own cells, and "autoreactive T-cells" that directly attack healthy tissues. The specific cells or tissues targeted determine the nature of the resulting autoimmune disease. For example, in type 1 diabetes, the immune system destroys insulin-producing beta cells in the pancreas; in multiple sclerosis, it attacks the myelin sheath that protects nerve fibers; and in rheumatoid arthritis, it primarily targets the lining of the joints.
Context
Autoimmunity represents a specific category of immune system dysfunction. While immunodeficiency involves an underactive or weakened immune response, and hypersensitivity (like allergies) involves an overreaction to harmless external substances, autoimmunity is a misdirected attack on the self. The precise causes of autoimmunity are not fully understood but are thought to involve a complex interplay of genetic and environmental factors. Individuals may have a genetic predisposition, often linked to specific human leukocyte antigen (HLA) genes, that makes them more susceptible. This susceptibility can then be activated by an environmental trigger, such as a viral or bacterial infection, exposure to certain chemicals, or even physical trauma.
Significance
For patients, an autoimmune diagnosis often means managing a chronic condition that can significantly impact quality of life. These diseases are characterized by periods of flare-ups, where symptoms worsen, and remission, where symptoms subside. There are more than 80 recognized autoimmune diseases, collectively affecting millions of people globally, with a notably higher prevalence in women. Understanding the mechanisms of autoimmunity is a major focus of medical research, as it is crucial for developing more effective diagnostics and treatments. Therapies often focus on managing symptoms and suppressing the overactive immune response, using medications such as corticosteroids, immunosuppressants, and targeted biologic drugs that interfere with specific parts of the immune pathway.
